Your Application may be rejected if you violate the following:

• If the names on your birthcert or old passport or ID cards do not tally with the one on your application it will be outrightly rejected.
• If you happen to be a first timer applicant with a non-bimetric birthcert, be sure your application will not be processed.
• If you have had any change of name please ensure you visit the PAC with your affidavit to avoid unnecessary delay or embarrassment.
• If your application happens to be unsigned, or unstamped be sure your application will not be processed and you will have to reschedule.
• Ensure your Witness is a Recognised legal practitioner, Licensed medical officer, Head teacher, or a Religious leader. Please do not confuse a registered Professional with a registered business.
• Note: The Ministry is very stern and bent on ensuring that the witness you have provided meets the Witness requirement. Kindly comply to avoid unnecessary delay.
